If you are a small business owner or non-profit and received a Paycheck Protection Program Loan, you may be eligible for loan forgiveness for a portion of, or the entire amount of the loan.
3508S FORGIVENESS APPLICATION
This version can only be used by businesses that received a loan of $150,000 or less.

3508EZ FORGIVENESS APPLICATION
If your loan is greater than $150,000, please first review the instructions for the PPP Loan Forgiveness Application Form 3508EZ to see if you qualify to use it.
If you qualify to use this form, please fully fill out the application, and provide us with the supporting documentation listed at the top of page 8.

Please be aware that after Clearwater Credit Union submits your application to the SBA, they have 90 days to remit the forgiveness payment or request additional information from the credit union. When we receive the payment, we will apply it to your loan and notify you.
